Waiver & Release

In consideration for allowing my child to participate in Hi Plains Youth 6-Man Football, I acknowledge and agree as follows:

Assumption of Risk

I understand that football is a contact sport involving risks including collisions, falls, concussions, head or spinal injuries, broken bones, heat illness, permanent disability, and death. I knowingly and voluntarily accept these risks on behalf of myself and my child during practices, games, conditioning, travel, and other league-related activities.

Release of Liability

I release and agree to hold harmless Hi Plains Youth 6-Man Football and Claude Youth Sports, along with their participating towns, organizations, directors, coaches, volunteers, officials, sponsors, facility owners, employees, agents, and representatives, from claims arising from my child's participation, including claims caused in whole or in part by their ordinary negligence. This includes, without limitation, negligent coaching, supervision, equipment selection or fitting, field conditions, failure to warn, or emergency response. This release applies to claims I may have individually and, to the fullest extent permitted by Texas law, claims asserted on behalf of my child. It does not release gross negligence or intentional misconduct.

Medical Authorization

If I cannot be reached during an emergency, I authorize league personnel to obtain emergency medical evaluation, ambulance transportation, hospitalization, and treatment for my child. I accept responsibility for medical expenses not covered by insurance.

Photo & Media Release

I give permission for photographs and video of my child taken during league activities to be used for league communications, results, websites, social media, and promotion without compensation.

Certification

I certify that I am the child's parent or legal guardian, that my child is physically able to participate, and that I have read and voluntarily agree to these terms. If any provision is found unenforceable, the remaining provisions will remain in effect. Texas law governs this agreement.

By checking the waiver box on the registration form, I confirm I have read, understood, and agree to this Waiver & Release.
